Most People With Malocclusion Don’t Know They Have It

Malocclusion — the clinical term for a misaligned bite — is among the most common dental conditions in the population. Research estimates that approximately 74% of Americans have some degree of malocclusion. A meaningful portion of those individuals have never received a formal diagnosis, because the presentation either wasn’t obvious visually or because their dental appointments focused on teeth and gums without a comprehensive bite assessment.

The reason malocclusion goes undetected is that the jaw is highly adaptive. Over years of living with a bite that doesn’t close correctly, the jaw muscles and joint structures compensate — finding the path of least resistance to closure that distributes chewing forces across the available tooth contact. This adaptation is so effective that patients often have no conscious awareness that their bite is off. They may notice occasional jaw soreness or fatigue, morning headaches that they attribute to stress, or mild sensitivity in certain teeth — but they don’t connect these symptoms to the bite because nothing in their daily experience has told them their bite is the issue.

The orthodontic evaluation is often where this connection is made for the first time.

What a Misaligned Bite Actually Does Over Time

The consequences of untreated malocclusion accumulate gradually, which is part of why they go unrecognized. There is no dramatic single event — just a slow progression of wear, compensation, and structural change that becomes significant over years and decades.

  • Accelerated tooth wear. When teeth don’t meet evenly, the teeth that do make contact bear disproportionate force. Over time, this produces accelerated wear on specific tooth surfaces — the flattening of cusp tips, the thinning of incisal edges, the loss of natural tooth structure in areas that receive excessive load. By the time this wear is visible, it has often been progressing for years.
  • Gum recession at specific locations. Excessive force on individual teeth from occlusal imbalance can contribute to gum recession at those sites — the gum tissue retreating in response to the force being transmitted through the root into the surrounding bone. This recession, like tooth wear, is not easily reversible once it has occurred.
  • Jaw joint and muscle strain. The temporomandibular joint functions best when the bite provides balanced, symmetric support for jaw closure. When bite imbalance persists, the joint compensates — often by shifting slightly from its optimal position, placing the disc and condyle in a relationship that produces the clicking, popping, and eventual soreness associated with temporomandibular dysfunction. Many patients who present to Dr. Iaculli with TMJ symptoms have an underlying bite problem that has been loading the joint asymmetrically for years.

Specific Bite Problems and What They Mean

There are several common bite presentations that often go undiagnosed because their visible manifestation is subtle while their functional consequences are significant.

  • Deep overbite. A deep vertical overbite — in which the upper front teeth cover too much of the lower front teeth when the mouth is closed — is one of the most commonly underdiagnosed bite problems. Patients are typically unaware of it because the teeth look reasonably straight. Clinically, a deep overbite places the lower front teeth in contact with the palate or the gum tissue behind the upper front teeth, contributing to gum tissue wear, excessive incisal wear, and jaw position problems.
  • Crossbite. A crossbite — in which one or more upper teeth bite inside the lower teeth rather than outside — produces lateral jaw shift and asymmetric bite forces that are invisible to the casual observer. The jaw shifts to one side to close, the muscles on one side work harder than the other, and over time structural asymmetry develops in the bone and musculature. Crossbites are among the bite problems that benefit most from early intervention while growth is still available to guide the correction.
  • Anterior open bite. An open bite — in which the upper and lower front teeth do not make contact when the back teeth are together — produces compensatory tongue posturing and concentrates all chewing forces on the posterior teeth, where enamel is thinner. Patients often have no idea they have an open bite because they’ve adapted their swallowing and chewing patterns to accommodate it.
